2019-20 Insurance Questionnaire
Physical Exam Form
Interested students should meet at the Merritt Truax Indoor Center Thursday, Sept. 12 at 10:45 a.m. Students should bring workout clothes, training / running shoes, and cleats (helmets and pads will not be needed).
Requirements to try out:
- Must have physician completed physical exam, including sickle cell test results, insurance and medical history form.
- Must be a full-time enrolled student in good standing at Oregon State University with a minimum 2.75 OSU GPA or 3.0 HS GPA for incoming freshmen students.
- Must have NCAA Division I eligibility remaining.

In order to participate, a scanned copy of your physical, insurance and medical history forms, along with a screen shot with your university status MUST be emailed to Vince Guinta at vince.guinta@oregonstate.edu by 12 p.m. on Sept. 10.
