Physical Exam Form

Interested students should meet at the Merritt Truax Indoor Center Thursday, Sept. 23 at 11:30 a.m. Students should bring workout clothes, training / running shoes, and cleats (helmets and pads will not be needed).

Requirements to try out:
- Must have physician completed physical exam, including sickle cell test results, insurance, and medical history form.
- Must be a full-time enrolled student in good standing at Oregon State University with a minimum 2.75 OSU GPA or 3.0 HS GPA for incoming freshmen students.
- Must have NCAA Division I eligibility remaining.
- Vaccination Status, if you are not vaccinated must have a negative PCR test within 48 hours of the Tryout

In order to participate, a scanned copy of your physical, insurance and medical history forms, along with a screen shot with your university status (Including OSU ID) MUST be emailed to Darrick Yray (darrick.yray@oregonstate.edu) by 12 p.m. on Sept. 17^th.
